Subject: Notification regarding the "Report Form for Personnel Returning from the Mainland" to be retained by the integrity units or personnel handling integrity affairs of each agency and school starting from February 27, 109.

Explanation:
1. In accordance with the Taoyuan City Government Integrity Office's official document dated March 12, 109, No. 1090001208.
2. This matter has been coordinated with the Personnel Office of the Taoyuan City Government. Starting from February 27, 109, the "Report Form for Personnel Returning from the Mainland" of each agency and school will be uniformly retained by the integrity units or personnel handling integrity affairs of each agency and school.

3. Please have the personnel handling integrity affairs at each school comply with Article 10 of the "Regulations Governing the Entry of Civil Servants and Specific Personnel from the Taiwan Area into the Mainland Area" and Article 4 of the "Key Points for the Operation of Civil Servants and Police Personnel at Rank 10 or Below, and Below Rank 4 of Police Supervisor, Who Do Not Involve National Security Interests or Secrets When Traveling to the Mainland Area" to continue to cooperate in handling the following matters:
(1) According to the "Regulations Governing the Entry of Civil Servants and Specific Personnel from the Taiwan Area into the Mainland Area" and the "Key Points for the Operation of Civil Servants and Police Personnel at Rank 10 or Below, and Below Rank 4 of Police Supervisor, Who Do Not Involve National Security Interests or Secrets When Traveling to the Mainland Area," personnel applying to enter the mainland area should fill out the "Report Form for Personnel Returning from the Mainland" within seven working days after returning to Taiwan and submit it to their subordinate agencies (organizations). The head of the agency shall submit it to the direct superior agency for record. If the content of the aforementioned report form is incomplete or raises doubts, the personnel returning from the mainland may be required to make corrections within a specified period; explanations may be requested when necessary.
(2) Please review monthly whether the application for personnel traveling to the mainland and the return report status are consistent, and fill out the "List of Personnel Entering the Mainland Area" for the head's reference. The aforementioned list should be sent to the Integrity Office of this bureau (email: 10025731@mail.tycg.gov.tw) by the 25th of each month for compilation (no reply is required if there is no data for that month).

(3) The format of the "Report Form for Personnel Returning from the Mainland" has been revised. Please fill it out in accordance with the revised format.
4. For agencies (organizations) and schools that do not have dedicated integrity personnel in this government, the signature field for the integrity unit on the "Application Form for Civil Servants Traveling to the Mainland" does not require a signature.
5. Attached are the "Comparison Table of Amendments to the Regulations Governing the Entry of Civil Servants and Specific Personnel from the Taiwan Area into the Mainland Area" (Attachment 1), "Amendments to Certain Provisions of the Key Points for the Operation of Civil Servants and Police Personnel at Rank 10 or Below, and Below Rank 4 of Police Supervisor, Who Do Not Involve National Security Interests or Secrets When Traveling to the Mainland Area" (Attachment 2), "Report Form for Personnel Returning from the Mainland" (Attachment 3), and "List of Personnel Entering the Mainland Area" (Attachment 4) for your reference.

列事項：
(一)依「臺灣地區公務員及特定身分人員進入大陸地區許可
辦法」及「簡任第十職等及警監四階以下未涉及國家安
全利益或機密之公務員及警察人員赴大陸地區作業要
點」規定，申請進入大陸地區之人員，應於返臺後七個
工作日內填具赴陸人員返臺通報表，送交所屬機關
(構)，機關首長送交直屬上級機關備查。前開通報表內
容如有未盡完整或疑慮者，得要求赴陸返臺人員於一定
期間內補正；必要時得請其提出說明。
(二)請按月審核赴陸人員申請與返臺通報情形是否相符，並
填列「進入大陸地區人員一覽表」提供首長參處；前揭
一覽表請於每月25日前免備文傳送本局政風室(電子郵件
信箱：10025731@mail.tycg.gov.tw)彙辦（該月份無資
料者，無須回復）。
(三)「赴陸人員返臺通報表」格式已修正，請本校同仁
確依該通報表格式填寫。

四、另本府未設有專責政風人員之機關(構)學校，「公務員赴
陸申請表」政風單位核章欄位即無須核章。

五、檢附「臺灣地區公務員及特定身分人員進入大陸地區許可
辦法修正條文對照表」(附件1)、「簡任第十職等及警監四
階以下未涉及國家安全利益或機密之公務員及警察人員赴
大陸地區作業要點部分規定修正規定」(附件2)、「赴陸人
員返臺通報表」(附件3)及「進入大陸地區人員一覽表」
(附件4)供參。
