1. In accordance with the letter from the Directorate-General of Personnel, Executive Yuan (hereinafter referred to as DGPA) dated May 22, 2023, No. 11200161182, and with a copy of the original letter attached.

2. The promotional DM, application form (individual and family type), insurance premium (credit card) automatic deduction authorization form, Q&A, and other related materials for this Plan have been announced on the DGPA's official website (https://www.dgpa.gov.tw/) in the latest news section, the e-Government Welfare Platform, the "Welfare & Recreation" section of the Compensation and Benefits Division, and the Fubon Insurance Civil Servant Travel Accident Insurance Card website (https://www.fubon.com/hwc); inquiry hotline: 0809-019-888.

3. This Plan will continue to be managed by Fubon Insurance. To protect the rights and interests of civil servants who have already applied for the Travel Accident Insurance Card, the company will send out separate notification letters to inform existing members of the plan details, which is hereby stated.
