For your information, the Taoyuan City Government's 114th year "Health Gatekeeper - On-site Health Checkup Activity" is being held. All faculty and staff are welcome to participate.

Details:
I. To encourage staff to prioritize their health management, enhance the effectiveness of health checkup subsidies, and provide convenient health checkup services, the Taoyuan City Government has planned the aforementioned health checkup activity for the 114th year. Taipei Veterans General Hospital Taoyuan Branch has been selected as the contracted medical institution through public selection. The details are as follows:
(I) Health Checkup Sessions and Times:
1. First Session: July 17, 114 (Thursday), 8:00 AM to 12:00 PM.
2. Second Session: July 18, 114 (Friday), 8:00 AM to 12:00 PM.
(II) Health Checkup Location: 4th Floor Auditorium and Parking Lot of Taoyuan District Office (X-ray vehicle, cancer screening vehicle).
(III) Eligibility: Limited to faculty and staff of all agencies (schools) under the Taoyuan City Government. Employee dependents are not eligible to participate. Service agencies (schools) are requested to grant official leave accordingly.
(IV) Health Checkup Packages: This health checkup offers three discounted packages: a NT$4,500 comprehensive package, a NT$7,000 premium comprehensive package, and a cardiovascular package. These include tumor marker screening, ultrasound, fundus photography, and other common disease-related examination items.
(V) Other: If eligible for the Taoyuan City Government's health checkup subsidy, you may apply for reimbursement of expenses to your service agency (school) with the payment receipt.

II. The aforementioned health checkup activity requires online registration. Related details are as follows:
(I) Registration Method (Please choose one session to attend. Do not register multiple times):

2. Second Session (July 18, 114, Friday) Registration Link: https://forms.gle/pLzwGPE12JEDf79m9, maximum capacity of 200 people.
(II) Registration Period: From now until June 25, 114 (Wednesday) at 5:00 PM, or until full.
(III) The Taoyuan City Government's Department of Personnel plans to notify successful registrants and their respective agency (school) personnel units via email on July 1, 114. If you do not receive a notification, please do not attend as on-site registration will not be available on the day of the event. If there are remaining spots, information regarding a second round of registration will be announced later.

III. Breast cancer screening (for women aged 40-74, subsidized once every 2 years) and cervical cancer screening (for women aged 30 and above, subsidized once annually), which are eligible for government subsidies, are scheduled for the second session (July 18, 114, Friday). This examination requires a minimum of 30 participants to proceed. If the number of registrants is insufficient, the service cannot be provided. Eligible faculty and staff are encouraged to participate.
