說明：
一、依桃園市政府人事處案陳行政院人事行政總處114年8月26日總處給字第1144001669號函辦理，並附原函及附件影本各1份。
二、為利同仁得於線上隨時查閱自身健檢資料，得簽署同意書，提供其實施健康檢查醫療機構，俾據以上傳健康檢查資料。
三、檢附衛生福利部統整健康檢查資料上傳相關問答集1份。
