| Subject: | To inform you that our city will be offering Moderna bivalent vaccine (BA.4/5) at vaccination stations and health center clinics on November 19, 2022 (Saturday) from 8 AM to 12 PM. Please be advised. |
| Details: |
| 1. | The Moderna bivalent vaccine (BA.4/5) is approved for use as a booster dose for adolescents and adults (12 years and older) who have completed their primary COVID-19 vaccination series. It can be administered at least 12 weeks (84 days) after completing the primary series or the previous booster dose. |
| 2. | Our city will be offering Moderna bivalent vaccine (BA.4/5) at vaccination stations and health center clinics on November 19, 2022 (Saturday) from 8 AM to 12 PM at the following locations. Please inform eligible faculty, staff, and students to take advantage of this opportunity: |
| (1) | Vaccination Stations: Ministry of Health and Welfare Taoyuan Hospital, St. Paul's Hospital, Chung Mei Hospital, and Chang-Jung Christian Hospital (4 stations). |
| (2) | Health Center Clinics: 13 district health centers offering walk-in vaccination services. |
| 3. | Attached are the "Vaccination Station and Health Center Clinic Information Card" and the "Vaccine Information, Assessment, and Consent Form." Related information can also be found on the homepage of our city's COVID-19 Prevention Information Station (https://covid-19.tycg.gov.tw/) > Vaccine Administration > Contracted Institutions Query. |
