| I. | In accordance with the letter Ref: No. 1130135650 dated November 18, 113, from the Department of Secondary Education, Ministry of Education. |
| II. | To encourage elementary and junior high schools to actively promote oral health work and develop school-based oral health plans for other schools to adopt, promote, and observe, the aforementioned plan is hereby organized. |
| III. | How to participate in the aforementioned plan: Please fill out the recommended list (Attachment 1) and the school self-assessment form (Attachment 2) by January 3, 114 (Friday). Submit the Word file (no seal required) and the scanned file of the sealed self-assessment form via email to: 10032273@ms.tyc.edu.tw, so that our bureau can select and recommend outstanding schools (the list will be notified separately). |
| IV. | Additionally, schools recommended by our bureau must upload the 113 academic year outstanding oral health school selection criteria form (Attachment 3) to the cloud space provided by the plan's review committee (URL to be provided separately) by 5:30 PM on February 14, 114 (Friday). |
