1.4/9After vaccination, theHPVvaccination card(pink)will be returned. Please keep it properly.

2.Grade 8 students(under 15 years old):Publicly fundedHPVvaccination2doses to complete.The protection lasts for about10years. Further vaccination will be assessed based on needs as they grow older.

3.If you are unable to get vaccinated at school on4/9due to any reason, please come to the health center to apply for a make-up vaccination notice to get vaccinated at the district health center.

4.Students getting the1stdose at school on4/9, after 6 months(after 10/9/115),parents should accompany them and bring thevaccination cardandNational Health Insurance cardto thedistrict health centerfor the2nddose!!

5.Students who received the1stdose at the health center,after 6 months from the vaccination,parents should accompany them and bring the vaccination card and National Health Insurance card to the district health center for the2nddose!!(No need to get a make-up notice from school)
