Regarding the health check discount program for civil servants and employees of Taoyuan City Government in 108, provided by Chang Gung Memorial Hospital of Linkou, Chang Gung Medical Foundation, and 6 other medical institutions, colleagues are welcome to refer to and utilize it.

Description:

1. To encourage colleagues to prioritize their self-health management and undergo regular health check-ups, Taoyuan City Government has actively negotiated with medical institutions of regional hospital level or above in the city to offer discounted health check-up programs, providing diverse choices for colleagues.

2. Linkou Chang Gung Memorial Hospital, Chang Gung Medical Foundation, and 6 other medical institutions have agreed to provide this discounted health check-up program. The relevant details are as follows:

(1) Health Check-up Programs:

1. Linkou Chang Gung Memorial Hospital, Chang Gung Medical Foundation: Offers two health check-up programs at NT$3,500 and NT$7,000 respectively.

2. Kaohsiung Chang Gung Memorial Hospital, Chang Gung Medical Foundation, and Taoyuan General Hospital, Armed Forces, Outpatient Department: Offers a health check-up program at NT$16,000.

3. St. Paul's Hospital, Sisters of Charity of Sts. Vincent de Paul Medical Foundation: Offers two health check-up programs at NT$3,500 and NT$16,000 respectively.

4. Shin Kong Wu Ho-Su Memorial Hospital, United General Hospital, and St. Mary's Hospital, Tiancheng Medical Foundation: Each offers three health check-up programs at NT$3,500, NT$7,000, and NT$16,000 respectively.

5. Some hospitals also offer health check-up programs at other price points. Please refer to the attached documents for details.

(2) Eligible Participants: All colleagues from agencies and schools under the Taoyuan City Government (including civil servants, contract employees, technicians, janitors, drivers, sanitation workers, survey assistants, and temporary staff), with no age restrictions.

(3) Validity Period: From now until December 31, 108.

3. Colleagues are encouraged to schedule their health check-ups early. If you have any questions regarding the health check-up programs offered by the aforementioned medical institutions, please contact the respective medical institutions directly.

4. Attached are the "List of Discounted Health Check-up Medical Institutions for Taoyuan City Government Civil Servants and Employees in 108" and one copy of the "Health Check-up Programs of this Medical Institution."
