The Taoyuan City Government's "Amazing Father, You're the Best" series of events, specifically the "Cherish Fathers, Guard Health" on-site health check-up activity, will be held on August 8th and 9th, 108 (2019). All colleagues from our school are welcome to participate.

Those interested in participating, please register with the Human Resources Office before the end of the workday on July 18th.

Details:

1. In accordance with the Taoyuan City Government's 108 (2019) "Amazing Father, You're the Best" series of events implementation plan and the 108 (2019) "Cherish Women" and "Cherish Fathers" on-site health check-up activity implementation plan.

2. To encourage colleagues to prioritize their health management, the Taoyuan City Government has partnered with the Far Eastern Memorial Hospital (a medical center) of the Yuan T. Lee Memorial Foundation to organize on-site health check-ups. These will be held on August 8th and 9th, 108 (2019), from 8 AM to 12 PM in the Grand Auditorium on the 4th floor of the Taoyuan District Office.

3. On the day of the event, discounted health check-up packages will be available for NT$3,500 and NT$500. Additionally, in conjunction with Father's Day (August 8th), male colleagues are encouraged to cherish themselves, stay away from smoking, and protect their health. Free smoking cessation clinics will also be offered, with carbon monoxide testing services provided by the Taoyuan City Government Department of Health. Participants who attend the smoking cessation clinic consultation on the day will receive a complimentary gift.

4. This event is open to all colleagues (including civil servants, contract employees, technicians, laborers, drivers, cleaning staff, survey assistants, and temporary staff) of all agencies, organizations, and schools under the Taoyuan City Government. Both male and female civil servants are welcome to participate. Their family members can also enjoy the same discounts. Furthermore, those who are eligible for the 108 (2019) health check-up subsidy from the city government can apply for the subsidy from their respective agencies or schools by presenting their payment receipts.

6. The Taoyuan City Government will arrange the examination dates for colleagues based on the registration status of each agency, organization, or school. The schedule will be announced on the internal network of the city government's Personnel Department after the registration deadline. Agencies, organizations, and schools are requested to assist in informing their staff of their scheduled examination dates and to remind them of health check-up precautions, such as fasting for 8 hours prior to the examination. To avoid disrupting public services, if a large number of staff from an agency, organization, or school plan to participate, please coordinate their examination times accordingly.

7. Attached are the registration form for the "Cherish Fathers, Guard Health" on-site health check-up activity, the precautions, and the item lists for the NT$3,500 and NT$500 health check-up packages, each provided as one copy.
