Taoyuan Junior High School Faculty and Staff COVID-19 Second Dose Vaccination Status Survey - Link

In accordance with the Taoyuan City Government Education Bureau's letter No. 1100078763 dated August 31, 110 (2021), to implement epidemic prevention work and monitor epidemic dynamics, all schools are requested to report the vaccination status of their full-time faculty and staff and the situation of students on campus (including the number of students attending school, leave status, home isolation, home quarantine, enhanced self-health management, self-health management, overseas stays, and return to Taiwan for schooling, etc.) to the Education Bureau's "COVID-19 Epidemic Reporting System" before 10 AM every school day.

In order to cooperate with the Student Affairs Office's Health Section in reporting the vaccination status of our school's full-time faculty and staff, if any colleagues have received their "second dose" of the COVID-19 vaccine (a total of 6 people have completed their second dose so far), please assist in immediately filling out the second dose vaccination status survey form [the form URL is as above, including uploading the "Vaccination Record Card"], so that we can provide timely reports.
