Faculty and staff, please bring yourNational Health Insurance card for on-site vaccination (10/29/2021, 1:00 PM - 3:20 PM, Heping Building, Bu San Jia Classroom), and instructors can get vaccinated with their classes.

Faculty and staff who were not pre-registered will not be vaccinated. On-site additions are not accepted.

For those who were pre-registered after the initial surveyand have been registered,,if you are unable to get your flu shot on the day of campus vaccination due to unforeseen circumstances,, the Taoyuan City Government Education Bureau agrees that under the condition ofcovering your own class arrangements, you may get vaccinated off-campus. You will be grantedofficial leavefor the vaccination. You must present the " Catch-up Vaccination Notice" issued by the school (expected to be distributed after 11/01/2021) and proceed toany health center in this city for vaccination. Vaccine supply is limited and available on a first-come, first-served basis.
