1. The eligible recipients for health check-ups are limited to full-time civil servants and employees who are 40 years of age or older and have been continuously employed for at least one year (age is calculated as of December 31 of the previous year, excluding substitute teachers and all types of temporary staff). Furthermore, those on leave of absence without pay may not apply for this health check-up subsidy before returning to work.
2. This health check-up is generally limited toonce every two years (including government subsidies and self-funded participation), but50year-old civil servants and employees can apply once a year.
3. After the examination, pleasesubmit the original payment receipt from the medical institution (if it is a copy, please add a note stating it matches the original and sign it) to apply for the subsidy, and according to regulations, the subsidy will be granted based on the actual amount within the limit ofNT$4,500 per person (50year-old civil servants and employees can receive NT$7,000 every two years or NT$3,500 annually), and any excess amount shall be borne by the applicant.4. Participants in the health check-up may register for
official leave, limited totwo days; teachers are responsible for their own class arrangements, and staff duties should not be affected. Please fill out a separate leave application form for approval.
