Target Audience:8thgrade girls (requires parental consent checked 'agree' on the consent form)

Female students in the 2nd year of junior high school in the 113 academic year who are citizens of this country and have never received the 9-HPV vaccine (i.e., female students who entered junior high school in 112, born between September 2, 99 and September 1, 100).

Vaccination Date:113September 27th (Friday) from 08:20 to 09:30 AM.Vaccination will be administered according to class assignment.

★On September 27th, be sure to bringyour health insurance cardto school.

★If you are unable to get vaccinated on that day due to unforeseen circumstances,please have the student obtain a supplementary vaccination notice from the health center,and thenparents must accompany the student to the public health center to receive the HPV vaccine.
